How to set up My Health Access Account (new user)

• Go to chgmd.com website
• Hit “Book appt”
• This will take you to the send web page — Catalyst Kinetics Medical Booking Page
• If you never made a My Health Access account before, hit the “sign up” icon
• If you have a My Health Access account, hit “book an appointment” icon
  • This will take you to the MyHealth Access web page
  • Enter in your email address and password
  • If you forget your password, please request a reset through the My Health Access web page
  • We do not have the ability to reset your password. Nor do we have access to what your password is.
• We will receive a notification for a patient request, and within 24 hours, we will validate your request

Voila! Your My Health Access account is active and you are now able to book appointments 24 hours a day, 7 days a week. Keep in mind that there are appointments reserved daily for same day bookings so if you have an urgent concern that comes up overnight, try booking first thing in the morning.

How to Book an Appointment

• Go to ckgmd.com website and hit “book appt”
• OR download the My Health Access app from your app provider
• Log in to your account using your email and password. We can not help you with your password at all.
